The National Center on Domestic Violence, Trauma & Mental Health (NCDVTMH) is engaged in efforts to identify, assess, and share information on evidence-based and promising trauma-informed practices relevant to survivors of domestic and sexual violence. As part of these efforts, NCDVTMH conducted interviews with 45 programs or initiatives identified by their peers as engaged in innovative trauma-informed work with survivors of violence and their children. Thirty of the interviews were conducted with domestic and sexual violence advocacy programs. In addition, 15 interviews were completed with programs providing trauma-informed or trauma-specific services for refugees and survivors of torture, as information gathered from these interviews may be relevant for domestic and sexual violence programs.

The interviews were designed to gather comprehensive information about how programs are currently conceptualizing trauma-informed and trauma-specific work and how this translates into enhanced or improved services for survivors of domestic and sexual violence. The interviews also identified aspects of trauma-informed services that are especially meaningful to survivors and ways that programs are measuring outcomes and evaluating the impact of their work. We were also particularly interested in culturally specific approaches to trauma and healing, including collective approaches, community-based practices, and those that can be offered by advocates and/or by trusted community members. Taken together, the information gathered from these interviews provides valuable insights on myriad ways to support survivors of domestic and sexual violence who may have experienced multiple traumatic experiences over the course of their lives.

The key themes that emerged from these interviews are summarized within this report. This project is part of a larger effort to build an evidence base for trauma-informed advocacy services and to expand our notions of healing, resilience, and recovery in the face of ongoing domestic violence and other trauma.

This brief report discusses two stereotypes that negatively impact survivors of domestic violence and sexual assault in legal proceedings: the stereotype that women who have mental health conditions or substance abuse issues are not credible when disclosing abuse and the conception that women with these conditions cannot also be good parents. The report also discusses ways in which abusers may use these stereotypes to manipulate victims.

National Domestic Violence Hotline

This report describes two surveys of callers to the National Domestic Violence Hotline, one on mental health coercion and the other on substance use coercion. The surveys showed that both types of abuse were prevalent among hotline callers. Mental health coercion involves issues such as an abuser threatening to tell authorities that a victim is “crazy,” an abuser trying to manipulate a victim into questioning her own mental health, and an abuser preventing a victim from accessing mental health treatment. Substance abuse coercion involves issues such as an abuser pressuring or forcing a victim to use alcohol or drugs, an abuser threatening to report a victim’s substance use to authorities as a manipulation tactic, and an abuser preventing a victim from accessing substance abuse treatment. The report explains that mental health and substance abuse practitioners and law enforcement and legal professionals need to be aware of these issues in order to work effectively with victims who have undergone these types of coercion.

This literature review provides an analysis of nine trauma-based treatments specifically designed or modified for survivors of domestic violence, along with caveats and recommendations for research and practice going forward.

Key findings included the following: Trauma treatments that have been modified to meet the specific needs of IPV survivors hold promise in helping women recover and successfully move on with their lives. Additional research is needed to address the domains not examined by these studies, particularly for survivors who have experienced multiple forms of trauma and for survivors from culturally specific communities. While it is too early to know definitively which treatments work best for which survivors, evidence suggests that helpful components may include (1) psychoeducation about the causes and consequences of IPV and its traumatic effects; (2) attention to ongoing safety; (3) cognitive and emotional skill development to address trauma-related symptoms and other life goals and concerns; and (4) a focus on survivors’ strengths as well as cultural strengths on which they can draw.

"In the first section of this report, Concerns of State Domestic Violence Coalitions and Concerns of Mental Health Providers and/or Agencies are described. The information in these sections emerged in the process of gathering information from State Domestic Violence Coalitions, State Mental Health Departments, community collaborations, individual programs, and from practitioners and experts across the country. The report then describes Model Initiatives and Programs that have already begun the work of examining and responding specifically to the mental health needs of battered women and their children. Many of these models are broadly conceived to respond to the mental health needs of trauma survivors in general, a group which includes survivors of domestic violence as well as survivors of other types of trauma, such as rape and childhood sexual abuse. Finally, the report reviews a number of well-respected training curricula that could be utilized or adapted to prepare advocates and mental health/substance abuse providers to respond to the complex needs of battered women and their children who are struggling with trauma-related mental health concerns."

This 2003 report describes the initial focus and work of the Domestic Violence & Mental Health Policy Initiative (DVMHPI), a Chicago-based project that was founded in 1999. In 2005, DVMHPI launched the National Center on Domestic Violence, Trauma & Mental Health, with funding from the U.S. Department of Health and Human Services; Administration on Children, Youth and Families; Family Violence Prevention and Services Program. While the Center maintains a national focus, DVMHPI continues to carry on the local aspects of the work by building the capacity of Chicago-area service systems to provide accessible, culturally relevant, and DV- and trauma-informed services.
