Considering Inter-Generational Violence When Planning for Prevention (Webinar)

This webinar presented by Dr. Linda Chamberlain discusses children who have experienced Adverse Childhood Experiences (ACEs). ACEs are a risk factor for female victimization and male perpetration of domestic violence. The concept of trauma-informed parenting and empowering parents who have experienced ACEs to become confident in their own positive parenting abilities is discussed. The webinar is part of the Alaska Network on Domestic Violence and Sexual Assault (ANDVSA) 2013 webinar series on Addressing Barriers to Health and Safety. The webinar runs 60 minutes.

This document expands the scope of routine screening for IPV to include assessment for reproductive and sexual coercion. A trauma-informed, comprehensive approach to relationship violence that includes behaviors that interfere with patients reproductive health can improve the quality of care and reproductive health outcomes including higher contraceptive compliance, fewer unintended pregnancies, preventing coerced and repeat abortions, and reducing sexually transmitted infections (STIs)/HIV and associated risk behaviors.

This curriculum provides training, tools, and resources to help home visitation staff address the complex and sometimes uncomfortable issue of domestic violence. When it comes to promoting health and safety outcomes for women and children impacted by domestic violence, there is a methodology to effective assessment, primary prevention, and anticipatory guidance messaging during home visits. These tools have been designed to facilitate safety planning and supported referrals to domestic violence programs.

This guide focuses on the transformative role of the reproductive health care provider in identifying and addressing intimate partner violence (IPV) and reproductive coercion. The Guidelines offer a discussion on the magnitude of IPV and reproductive coercion; overview of the health impact; guidelines for implementing routine assessment and intervention; policy implications and system responses; and provides validated assessment tools. The Guidelines specifically examine how IPV and reproductive coercion impact contraceptive use and birth control sabotage; condom use; unintended pregnancy; and the role of pregnancy coercion, including exposure to STIs. The latest data in each of these areas is explored, in addition to identifying tools such as safety cards, posters and documentation forms to help launch health care responses. The goal of this resource is to reframe the way in which health care systems respond to intimate partner violence and reproductive coercion such that the reproductive health care provider is the hub in a wheel of a trauma-informed, coordinated health care response.

A large international literature promotes restorative justice options as satisfying and empowering to crime victims. This paper examines restorative justice for sexual assault from the perspective of three groups of survivors: (a) adults victimized by adult perpetrators; (b) adults or juveniles victimized by juveniles; and (c) adults sexually molested as children by adults.

A restorative justice conceptualization involves three constituencies: (a) survivor/victims and secondarily victimized family and friends who suffer distress along with their loved one; (b) community members who experience less safety and social connection when they perceive high levels of crime and low deterrence, yet who simultaneously may be contributing to an environment supportive of sexual violence; and (c) offenders as well as their families and friends, who experience guilt and shame that is associated with being accused of a sexual crime or belonging to the interpersonal relationship context from which the offense arose. Restorative options include sharing circles, victim-offender dialogue, victim impact panels, community reparation boards, circles of support, sentencing circles, conferencing with juveniles and adults, and restorative discipline in educational settings (Umbreit et al., 2006). Whereas judicial processes and incarceration primarily aim for deterrence and punishment of the offender, restorative justice can be both responsive to survivor needs for validation, empowerment, and repair of harm and preventative of future sexual assault.

This paper reviews data documenting the high levels of attrition that transpire between reporting sexual assault to law enforcement and case disposition by prosecutors. The data demonstrate that conventional justice has over a period of years been very good at doing little to respond to sexual assault reports. Some of the notable evidence of resistance to change in this system includes the lack of a positive trend despite dedicated activism and education/training in increasing reporting or conviction rates in sexual assaults. Even when restorative alternatives are made available, the status quo frequently prevails. Program directors report that the rates of utilization by prosecutors are low.

Survivor/victims have many justice needs that are currently unmet by conventional justice. Compared to current practice, groups that could receive services from restorative justice programs include: (a) adult survivor/victims who choose not to initiate a report to law enforcement; (b) those who reported but whose cases were closed against their wishes; (c) survivor/victims whose offenders were never apprehended; (d) adult survivors of child sexual assault who were unable to come forward at the time of the offense; (e) juvenile survivor/victims whose offenders are young family members and friends where the offense was an overly extreme developmentally driven sexual exploration occurring in the absence of evidence of deviant sexual arousal; and (e) family and friends of survivor/victims, who may suffer equal or more distress than the survivor/victims. There is a credible argument that sexual assault service providers, advocates, and policy makers can take leadership in the development of restorative options both in parallel with and independent from the conventional justice system. Meeting survivor/victims' justice needs and fostering accountability is preventative in itself and bolsters the primary prevention messages that are central elements of rape prevention education programs. Innovation from both inside and outside the justice system would benefit all parties.

Exposure to violence over the lifespan is very common and is associated with long-term health problems, increased risk of revictimization, and intergenerational violence. Assessment for lifetime exposure to violence can identify unknown, unresolved histories of victimization and lead to detection, early intervention, and prevention of predictable health consequences and future violence. This paper examines how assessment for lifetime exposure to violence can create a pathway to prevention and address the long-term consequences of exposure to violence over the lifespan.

For the purpose of this discussion, lifetime exposure is limited to childhood abuse, childhood exposure to domestic violence, domestic violence, and lifetime physical and sexual abuse. Retrospective studies that ask adults to recall past traumatic events have revealed that these forms of victimization are extremely prevalent. A growing body of research has documented similar patterns of chronic diseases, risk behaviors, and other health problems that are highly correlated with these different forms of victimization (Felitti et al, 1998; Goodman et al, 1993; Springer et al, 2003). An often ignored outcome of lifetime exposure to violence is the elevated risk of revictimization, perpetrating violence, and transmitting violence to the next generation (Arias, 2004; Avery et al, 2002; Noll, 2005; Whitefield et al, 2003).

The long-term adverse adult outcomes associated with past victimization make a strong case for addressing lifetime exposure to violence in a wide range of service settings. Assessment for lifetime exposure to violence in the health care setting can be a starting point to develop a community-wide response. Questions for lifetime exposure to violence can be integrated into routine health histories and self-administered patient assessment forms. Research suggests that survivors want to talk about their past experiences in a supportive, safe environment (Rhodes, 2005; Whiteman, Chamberlain & Greenwood, 2004).

Making the connection between past victimization and chronic diseases provides a unique opportunity to identify hidden risk factors and provide more effective case management for patients' physical and psychosocial needs. Identifying past victimization also creates opportunities for early intervention and prevention of long-term adverse outcomes. This paper introduces a conceptual model called the prevention continuum that builds upon traditional prevention theory to identify ongoing opportunities for multi-level prevention strategies to address the effects of violence over the lifespan and prevent future violence. This approach creates the opportunity to build 'prevention-readiness' into prevention activities by addressing how past victimization can impact the effectiveness of prevention strategies. The prevention continuum is survivor-inclusive, meaning that survivors are included in all levels of prevention (primary, secondary and tertiary). Examples are provided to demonstrate how providers who assess for lifetime exposure to violence have an important role in primary prevention.
