Treatment Regimen and Medical Case Management

The Institute for Healthcare Improvement defines medical case management (MCM) services as a range of client-centered services that link clients with healthcare, psychosocial support, and other services. The goal of medical case management is to improve treatment adherence and health outcomes for clients through linkages to community services, navigation of the health system, treatment promotion, retention in care, harm reduction, and increased social support. Below are tools to help domestic violence programs think about and design formal partnerships with HIV programs. Advocates, case managers, and domestic violence service providers should develop meaningful relationships with AIDS Service Providers in the community and continue to partner to provide the most comprehensive and safest services for survivors impacted by HIV/AIDS and their families.
